Membership Contract

This form serves as my application for membership at Wrench Fitness. In consideration of my being granted membership and thereby allowed access to
the premises and use of the equipment of Wrench Fitness, I agree:
1. That I am 18 years of age or older. If I am under the age of 18, I must have my parent or proper guardian sign this Agreement as indicated
below.
2. That I will abide by the gym rules posted. I agree that violation of the rules may result in loss of gym privileges and termination of
membership.
3. That I will use the gym, its premises and the equipment solely at my own risk. I understand that the activities I undertake at the Wrench
Fitness Inc. facility may not be supervised. It is my sole responsibility to monitor my own safety and well being and to seek assistance and/or
medical attention as necessary.
4. That I understand it is advised to obtain a doctor’s advice before commencing a work out regiment and before undertaking any physical
activity. I acknowledge that I am in no way relying on Wrench Fitness Inc. in relation to my health or ability to carry out physical activity at the
Wrench Fitness Inc. facility. It is my sole responsibility to ensure that I a physically fit to engage in any activities that I undertake as part of my
membership with Wrench Fitness Inc. and I assume full responsibility for consulting with my own physician about my participation in such
activities.
5. I am aware that participation in physical activity, including, but not limited to, running, biking, yoga, weight lifting and participation in fitness
classes, involve risks, dangers and hazards, including risk of serious physical injury and/or death. I freely accept, and fully assume all such
risks, dangers and hazards and the possibility of personal injury, death, property damage or loss resulting therefrom.
6. That I hereby release Wrench Fitness Inc., its officers, directors, employees, representatives and agents from and against all actions, causes of
action, suits, liability, claims, demands, damages, losses, costs and expenses (including, without limitation, those arising from any negligence,
misconduct, error or omission, in tort or contract) whether direct, indirect, special, exemplary or consequential (the “Claims”) arising out of
or in consequence of participating in physical fitness activities at the Wrench Fitness Inc. facility.
7. I hereby agree to indemnify, hold harmless and forever discharge Wrench Fitness Inc., and its directors, officers, employees, representatives
and agents from and against any and all Claims that may be incurred by Wrench Fitness Inc. if I suffer harm or loss, injure someone else or
damage property as a result of my participation in physical fitness activities at the Wrench Fitness Inc, facility, and my activities forming such
participation.
8. That I partake in fitness classes solely at my own risk.
9. That membership is non-refundable/no holds.
10. That personal training contracts are non-refundable after a period of 30 days.
11. All prices shown have 5% GST added to them
I am aware that persons over 45 years of age should have a medical check before engaging in an exercise program. I have read this application and
agreement.

-I understand and agree to the fact that my membership cannot be cancelled during my initial term choice and following my initial term, my membership becomes a perpetual monthly membership.
-After my initial term, or at any time during my perpetual monthly membership, I understand that two weeks’ notice must be given before cancellation will go into effect or before cancellation will be accepted.
-I understand and agree to the fact that if I choose to cancel my membership before my initial term has expired that I will be required to pay $35 per
month(s) of what is remaining on that term.
-I understand and agree to the fact that if one of my bi-weekly payments returns NSF, a $15.00 charge will apply on my next payment.
-I understand and agree to the fact that if my void cheque/banking information is not received within 2 weeks a $3.00 per day late charge applies.
-I agree that the above has been explained to me and I fully understand and accept responsibility for my membership at Wrench Fitness.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS DOCUMENT AND FULLY UNDERSTAND THE NATURE AND CONSEQUENCES OF SIGNING IT. I HAVE EXECUTED THIS DOCUMENT FREELY WITHOUT ANY INFLUENCE OR DURESS BY WRENCH FITNESS INC., ITS DIRECTORS, OFFICERS, EMPLOYEES, REPRESENTATIVES OR AGENTS OR ANYONE FOR WHOM IT MAY BE RESPONSIBLE. I VOLUNTARILY AGREE TO THE TERMS AND CONDITIONS STATED ABOVE. I WARRANT THAT I AM APPROPRIATELY FIT TO PARTICIPATE IN THE ACTIVITIES I WILL UNDERTAKE AS PART OF MY MEMBERSHIP WITH WRENCH FITNESS INC.

24hr Access Waiver

This form serves as my application for 24hr access at Wrench Fitness. In consideration of my being granted 24hr access and thereby allowed access to the premises and use of the equipment of Wrench Fitness, I agree:
1. That I am 18 years of age or older. If I am under the age of 18, I must have my parent or proper guardian sign this Agreement as indicated below.

9. That I fully accept responsibility for myself and my actions during non-staffed hours.
10. That allowing a non-member in, during non-staffed hours incurs an access freeze on my membership and a $50 charge to unfreeze my access to Wrench Fitness.
11. That for my SAFETY and SECURITY I will be monitored via CCTV at all times while on the gym premise/floor.
12. That I will return weights and/or equipment to their proper location after use.
13. That I will only use the EMERGENCY Services panic button in the case of an EMERGENCY. Any other use of the panic button will result in a $100 charge.

I HAVE EXECUTED THIS DOCUMENT FREELY WITHOUT ANY INFLUENCE OR DURESS BY WRENCH FITNESS INC., ITS DIRECTORS, OFFICERS, EMPLOYEES, REPRESENTATIVES OR AGENTS OR ANYONE FOR WHOM IT MAY BE RESPONSIBLE. I VOLUNTARILY AGREE TO THE TERMS AND CONDITIONS STATED ABOVE. I WARRANT THAT I AM APPROPRIATELY FIT TO PARTICIPATE IN THE ACTIVITIES I WILL UNDERTAKE AS PART OF MY MEMBERSHIP WITH WRENCH FITNESS INC.
